Advance Care Planning for Dementia Care:

  • Audience: Primary Care Clinicians for patients with dementia and their loved ones

  • Training Description: Refresher course for PCPs on communication skills in ACP for patients with dementia. It will cover the major stages of dementia and key communication skills for those stages.

  • Time length: 5 chapters, approx. 3 hours to complete total

  • Certificate of completion after each chapter

  • Generously funded by the Gordon and Betty Moore Foundation

Comfort First in Dementia Care Training:

  • Audience: All nursing home staff who care for people living with dementia

  • Training Description: Provides skills and new information to maximize the comfort and quality of life for persons with dementia.

  • Time length: 6 modules, approx. 3 hours to complete total

  • Certificate of completion after each chapter

  • Generously funded by the Centers for Medicare and Medicaid Services

Decision Aid for Goals of Care:

  • The Decision Aid for Goals of Care can be used by clinicians in nursing homes who are working with people with dementia and their family members.
  • This decision aid is first shared with family members in order to prepare and educate them about health care decisions for people with advanced dementia.
