Advance Care Planning for Dementia Care:

Audience: Primary Care Clinicians for patients with dementia and their loved ones

Training Description: Refresher course for PCPs on communication skills in ACP for patients with dementia. It will cover the major stages of dementia and key communication skills for those stages.

Comfort First in Dementia Care Training:

Audience: All nursing home staff who care for people living with dementia

Training Description: Provides skills and new information to maximize the comfort and quality of life for persons with dementia.

Time length: 6 modules, approx. 3 hours to complete total

Decision Aid for Goals of Care:
- The Decision Aid for Goals of Care can be used by clinicians in nursing homes who are working with people with dementia and their family members.
- This decision aid is first shared with family members in order to prepare and educate them about health care decisions for people with advanced dementia.
